What Is Primary Care?
Primary care covers a wide range of healthcare needs, including:
Preventive care – regular exams, screenings, and immunizations to keep you healthy and catch issues early
Diagnosis and treatment of short-term illnesses and injuries
Management of chronic conditions like diabetes, hypertension, asthma, and more
Health counseling and education to help you make informed decisions
Referrals to specialists, when needed, with coordination from your primary care provider
At its heart, primary care is about building a lasting relationship with your provider—someone who knows your history, listens to your concerns, and partners with you to maintain your best possible health.

Partnering with patients and their families for the long term requires understanding and respecting each patient’s needs, culture, and values. The Family Healthcare medical home practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core members of the care team, the provider will ensure that they are fully informed partners in establishing care plans.
Medication Assisted Treatment (MAT): The use of FDA approved medications, in combination with counseling and behavioral therapies for the treatment of substance use disorders. The prescription medication helps to normalize brain chemistry, block the euphoric effect of substances, relieve physiological cravings, and normalize body functions. The goal of MAT is full recovery, including the ability to live a self-directed life. This treatment approach has been shown to:
Improve patient survival
Increase patient retention in treatment
Increase patient ability to gain and maintain employment
Improve birth outcomes among women who have substance use disorders and are pregnant
Decrease illicit substance use and other criminal activities among people with substance use disorders
And more!
